PII is any data that could potentially identify a specific individual
PHI is any info (oral or recorded) in any form that can be used to identify an individual which is created or received by a health care provider, health plan, employer, life insurer, educating entity relating to past, present, future physical/mental health of any individual including future payment for health related services
Examples of PII, PHI, Proprietary Info:
PII: social security #, DOB, driver’s license, address, financial/banking info (credit/debit)
PHI: medical transcripts, medical billing info, test results, scans, appointment/scheduling info
Proprietary Business Info: intellectual property, financials, trade secrets, client lists, legal/litigation info, contracts, etc., or anything interpreted as being sensitive in nature to an entity
